filling out this questionnaire online or taking the questionnaire over the
phone.
If you're ready to complete your FEMA application, the
process takes about 20 minutes and can be completed on the FEMA website .

• Postal address and the place where the damage occurred
• Insurance information
• Annual household income
• Description of damages and losses
